4th Annual TCD Summit

To register, please complete and return the registration form with payment to the Continuing Medical Education office address listed at the bottom of the registration form.  You may also register by FAXing your registration form to the CME office at 573-882-5666, or by completing an online registration.

  • Payment Methods:

Registration fee may be paid by cash, check or any major credit card (Discover, MasterCard, Visa or American Express).  Checks should be made payable to University of Missouri.

  • Substitution and Refund Policy:

A full refund of fees less a $10 administrative fee will be made if notice of cancellation is received, in writing, prior to Friday, March 31, 2017.  No refunds will be made after March 31, 2017.
